Vista Hill Foundation Native American SmartCare is proud to partner with Sonoma County Indian Health Project Clinic, located in Santa Rosa, California and a satellite clinic in Point Arena, California. Working in direct collaboration with the health center providers, SmartCare is able to further provide emotional, behavioral, and mental health support in our comprehensive approach.

Sonoma County Indian Health Project offers Medical, Pharmacy, Dental, Behavioral Health, WIC and Nutrition Services, Diagnosis, and Treatment for acute and Chronic Medical conditions, Prenatal and OB Care, General Pediatric Care, Podiatry Services, and Specialty Services, such as Rheumatology, Gynecology, and Herbal Integrative Medicine, Preventative and Wellness Support Services, Phlebotomy Services, Health Education and Smoking Cessation Counseling, and Traditional Native Healer Services.
